Essure: Heart stent-like implants inserted into the fallopian tubes with a hysteroscope to provide permanent sterilization. Performed in-office or outpatient facility with local anesthesia. Takes 20-30 minutes. No incisions. 99.8% effective. Not reversible. Need other contraceptive for three months, then an x-ray dye test to confirm closure of tubes. Immediate return to routine activity on same day. Hysteroscope: A tiny telescope to view inside the uterus and diagnose the cause of abnormal bleeding. Done with local anesthesia. Takes approximately 15 minutes. Patients return immediately to routine activities. LEEP (Loop Electro surgical Excision Procedure): A tiny wire loop through which an electrical cutting current passes to allow removal of tissue from the cervix, vagina or vulva for diagnosis of disease. Used with local anesthesia. Essentially bloodless. Takes about 10 minutes and patients receive post op instructions on permitted activity. Colposcope: A magnifying scope to study the cervix, vagina, or vulva. Special solutions are washed over the tissue to highlight abnormal cells. Primary tool used to investigate abnormal pap smears. Biopsies may limit some activities for a short period of time. Cystoscope: A tiny telescope used to view inside the bladder. Performed with pre-medication and local topical anesthetic. Immediate return to usual activities. Urodynamics: A study of bladder function using a small catheter to record bladder activity as saline is slowly dripped in. Pressures are transferred to a graph and results assist in diagnosing bladder control problems. Takes about 15 minutes and routine activities may be resumed immediately. Requires no anesthetic. Ultrasound: Medical sonar used to evaluate growths on the uterus and ovaries and to verify and date pregnancies. No anesthesia necessary. Intrauterine Insemination: Infertility procedure in which the collected sperm is prepared with a special wash and injected directly into the uterine cavity to bypass hostile cervical mucus. Cryotherapy: Treatment of diseased tissue (usually the cervix) with a probe which freezes and destroys abnormal cells. Takes 1 to 3 minutes and only ibuprofen premedication is necessary. Return to work is immediate. Some watery discharge follows from one to three weeks. HOSPITAL SPECIAL PROCEDURES Operative Laparoscopy: Using a finger-sized telescope and small, half-inch incisions to perform hysterectomy, fibroid removal, endometriosis excision, tubal sterilization, appendectomy, and ovarian cyst removal to name a few. Requires general anesthesia. Laparoscopic Burch: A laparoscopic outpatient procedure to correct bladder neck support and urinary incontinence. Requires general anesthesia and a post-op catheter. LSH (Laparoscopic Supracervical Hysterectomy)
: Laparoscopic Operative Hysteroscopy: Outpatient use of the hysteroscope to view the uterine cavity and remove fibroids, polyps, uterine septae and adhesions to name a few. Requires general or local anesthesia with IV sedation. Endometrial Ablation: A hysteroscope operation in which the uterine lining is treated with heat to destroy the glands responsible for bleeding. Used as an alternative to hysterectomy to treat abnormally heavy uterine bleeding. Immediate return to routine activities post-op. Suburethral Sling: A sling to lift the bladder neck and correct urinary stress incontinence or a weak urethral sphincter. Placed through tiny incisions above the pubic bone. Requires IV sedation, general, or epidural anesthesia. A post-op bladder catheter and one night hospital stay is routine, but may also be done as an outpatient. Transobturator Sling (T.O.T.) : Similar to suburethral sling except this approach avoids potential injury to the bladder and the incisions are in the crease of the groin.
